It has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us.
Welcome to SystemOnline. Patients of Park Medical Centre can register to use our online services which include booking appointements, ordering prescriptions and viewing your summary care record and so much more.
To register for SystemOnline, please bring a valid photo ID i.e. passport or driving licence to the surgery and fill out a registration form. We can then give you your unique username and password, which will allow you to access all of our online services.

A number of patients attended our meeting on 24th March to discuss the progress made with regard to the caring for the patients in The Chiswick Nursing Centre. Two new letters were discussed, please these find below:

Report on Patient Forum 24th February
Over 50 patients attended the forum on the 24th February and we were able to report on some forward movement on the part of the CCG. We were particularly pleased to welcome relatives of patients in the nursing home. As before we are grateful for the sound advice and support that our patients gave us.
It is our hope to meet with the CCG shortly although no date has yet been set. We certainly hope that we will have had a meeting and something concrete to report before too long.
Please make a note of the provisional date for the next patient forum, Tuesday 24th March.
Four additional letters were discussed at the forum. A letter from the CCG and a reply from Park Medical Centre plus a short one from the CCG confirming that we will receive help from a consultant geriatrician. The other is from Ganymede Ltd, the company that runs the nursing centre. You can download these here.

Adopted Resolution at a meeting held on 29th January 2015.
The resolution has been turned into a petition and we will be collecting signatures in the surgery. A copy of the petition is attached. We would be grateful if you could collect signatures, if you can, and bring any sheets to the meeting on the 24th.
On the 29th January we were asked to provide a model letter for Jeremy Hunt, the health minister, for you to use. This is attached. Please amend and adapt as you wish. You can email him at the Department of Health on email@example.com.

You can access this by logging in to SystemOnline using your personal username and password.
