We have conducted a survey into patients' views of the service that we provide here at Park Medical Centre with the help of our Patient Reference Group. The survey we have been using closed at the end of February 2013. We hope to reopen a new online survey soon. We really value your views and you can pick up a paper copy of survey in the surgery.
We are offering free NHS health checks for patients aged 40 to 74 who do not have previously diagnosed cardiovascular disease, diabetes, or hypertension. The pre-appointment questionnaire is attached here. You will be contacted if you qualify or phone the surgery to make a booking.
GENERAL PATIENT INFORMATION
Patients are entitled to be treated fairly on the basis of medical need and not discriminated against in any way.
If communicating in English is a problem, bring a friend or family member to translate for you if possible. Please let us know if you need a translator and we will try to arrange one.
Keep it or cancel it:
Please help us to offer all our patients the best service we can. If you can no longer make an appointment please make sure you call to cancel with as much notice as possible. Please ensure that you arrive on time for an appointment. If you are running late please let us know and we will try and offer you options. If you are more than 10 minutes late the doctor may not be able to see you. We will try our best to let you know if a surgery is running late and your appointment likely to be delayed.
Medical certificates for sickness:
UK law allows workers to self-certificate for the first seven days of any illness. To obtain a certificate after seven days you must see a doctor.
Copies of Referral Letters:
If you would like a copy of any referral letter sent by the doctor on your behalf to the hospital please let the doctor know during the consultation. We will arrange for the copy to be available at reception for you to collect.
Data Protection & Confidentiality:
The Practice is very aware of confidentiality and data protection issues. We take such matters very seriously and have stringent confidentiality and data protection procedures, which are regularly reviewed.
Patient Information and Data:
Patient information is held by the doctor and accessible to the practice nurses, practice manager and reception. It is also accessible to the Hammersmith & Fulham Primary Care Trust.
All these parties may not disclose any identifiable information to any other outside agency without the patient’s agreement.
Patient data (anonymous) may be used for audit purposes.
There are plans for patient summary care records to be made available to other health care professionals. If patients wish to be excluded, they should contact the practice manager.
Under the Data protection Act 1998, you have a legal right to access your health records. If you would like to see your medical records please ask reception about the process.
As part of the Freedom of Information Act 2000 the practice has a publications scheme which outlines types of information we make available to the public (this does not include any patient information). A copy of the scheme is available. Please ask at reception.
From time to time we may text you with information e.g. additional flu clinics or to ask for medical information. Please let us know if you would prefer not to receive texts.
The Park Medical Centre supports the NHS Zero Tolerance policy with regard to anti-social, abusive or violent behaviour towards doctors or staff.
Health Information Leaflets:
Please note that there are a number of leaflets on various health issues available at the practice. These can be found in the waiting area. Please ask the receptionist or a nurse for any leaflets that you cannot find.
Feedback, Comments & Suggestions:
We are constantly trying to improve our standards of service. We welcome comments and suggestions on all aspects of the care we provide. We also understand that there are times when you may feel it necessary to complain.
Suggestions or complaints can both be made on the special slips located on the reception desk and placed in the box provided.
In addition we have a formal complaints procedure. Please ask to speak to the practice manager or send a letter to the surgery.
